Our community nursing team consists of two main services:
Clinical Nurse Specialists and the Hospice at Home Team
The hospice offers a range of community services to people in their own home dependant on their needs. Following your referral to St. Wilfrid's Hospice you will be assessed by a doctor or Clinical Nurse Specialist who will be able to advise you on the services available.
