Hospice Services

St Wilfrid's hospice welcomes referrals from other healthcare professions - see the referral guidelines for full details. Referrals may be made by letter, fax 01243 538171 or telephone call to 01243 755813 - such referrals should include all the detail requested on the referral form.

If a person is not in the St Wilfrid's catchment population there are also hospices in Portsmouth and Worthing and a community team covering the Midhurst area - see other local hospices for further details.
An electronic copy of the Wessex Palliative Care Handbook of clinical guidelines ("The Green Book") is available on this website, as is an opioid conversion guide to aid the conversion of one opioid to another. The hospice ward nursing team is able to provide out-of-hours telephone advice.

The hospice clinical team offers a full range of specialist palliative care services and includes the following professions:

Several local pharmacies stock medications commonly used in palliative care details of which are to be found under pharmacy information.
St Wilfrid's works closely with the PCT and other NHS colleagues to promote the implementation of the NHS End of Life Care Programme and uses tools such as the Gold Standards Framework and Liverpool Care Pathway.
